Attached is the official announcement and the official forms for the required submissions for RISE 2026. 

The Form 2 should be accomplished only if there are graduates from other institutions whom you will allow to take the RISE 2026 in your institution. Forms 1 and 2 are attached to the electronic mail sent to accredited training institutions.
